2008 Income Tax Check List

* A copy of your 2008 Income Tax Return (New clients only)
* All W-2's, 1099-R's, and any other form showing withholdings and/or income. (all copies)
* Copies of K-1's for partnerships and S-corps. (If applicable)
* Have any of your dependent children received any unearned income over $1600? (Sources: interest income, sale of stock, etc.)
* 1099-INT's (interest income from all bank accounts) and 1099-DIV's (dividend income form all stock, life insurance, etc.)
* Did you cash in any savings bonds? If yes, you will need to know amount of interest received.
* Gambling winnings, Jury Duty and any other miscellaneous income received. (Gambling losses are allowed up to gambling winnings claimed)
* 1099-G (Unemployment compensation)
* Alimony received or paid.
* Did you or are you planning on putting money in an IRA? There are income limitations on the amount deductible on your Federal Tax Return.
* If you sold stock, you will need to know your cost for the stock.
* Social Security Income
* If your child attended day care, you will need providers name and Fed ID# or Social Security Number along with the address of the day care location.
* Did you pay rent? Please provide landlord's name, address and amount paid in 2007.
* College Tuition for self and/or dependent. Please specify for whom the expenses pertain to and what year the student was enrolled in. You may be eligible to claim expenses for more than one person.
* Military families in combat zone should make me aware.

* Medical Deductions Includes: Health Insurance Premiums, Hospitalization, Medicare Insurance Premiums Paid, Contact Lens Insurance, Dental Insurance, Prescribed Drugs & Insulin, Doctors & Clinics, Phone Toll Charges for Medical Purposes, Dentists & Orthodontists, Glasses, Contact Lenses, Eye Examinations, Nurses, Alcoholism & Drug Treatment, Ambulance, Lab Tests, Therapy, X-Ray, Anesthesiology, Prescribed Medical Equipment, Corrective Devices, Thermometers, Vaporizers, Hearing Aids, Batteries & Related Equipment, Vasectomy, Tubal Ligation, Special Foods (if prescribed), Nursing or Retirement Home (Medical care only), Schooling for Handicapped, Cosmetic Surgery: (generally not deductible unless it corrects a congenital abnormality, a disfiguring disease or an injury), Medical Transportation, Lodging while obtaining Medical Treatment.
* Taxes Includes: State Income Taxes Withheld, State Income Taxes paid on Estimates, State Income Taxes Paid this year for prior years, Real Estate Taxes, Other Real Estate Taxes (2nd home, cabin, etc.), Excise Tax.
* Interest Paid Includes: 1st Mortgage Interest, 2nd Mortgage Interest, Home Equity and Home Improvement Loan Interest, Loan Points, Investment Interest Paid.
* Refinanced, Purchased, Sold Residence Provide closing papers.
* Contributions
Cash Contributions: Churches or Synagogues, United Campaign (include payroll deduction), Cancer, heart, Scouts Boy/Girl, M.S., M.D., March of Dimes, Food Shelf, Our of pocket expenses for Charitable work, Veteran Organizations, Salvation Army, Etc.
Non-cash Contributions: Fair Market Value of Items given to Charity. If over $500, provide receipt.
* Miscellaneous Deductions Union Dues and Professional Dues, Professional Licenses and Fees, Professional or Work Related Subscriptions, Journals, etc., Tools/Supplies purchase for job, Safety Equipment (helmets, shoes, protective clothing, etc.), Special Uniforms for Job and their Cleaning, Business Insurance, Telephone (Business line of long-distance toll calls only), Business Mileage (this includes business travel for employer; travel to professional meetings; travel from your residence to a temporary work location; travel between 1st and 2nd job on same business day; job seeking mileage; and educational travel between hob and school), Employment Related Schooling or Seminars (this includes: Travel, Air Fare, Lodging, Food, Employment Agency Fees, Resume, etc.), Tax Pre Fees, Financial Planning, Consultation Fees, Investment Expenses, Safe Deposit Box, Investment Publication & Journals, IRA & Keogh Fees you paid directly.
* Gambling Losses (only to the extent of Winnings claimed)

* Self-Employed or Sub-Contractors Gross Receipts - Provide all 1099 forms you have received
* Rental Income Total of Rents received per property. (If you purchased or sold rental property, you will need to provide closing papers)
